The Social Worker assists with psychosocial assessments, care planning, admissions and discharge planning, resident and family support, community resource coordination, resident rights, and addressing concerns or changes in psychosocial needs. Working closely with nursing, therapy, activities, providers, and other members of the interdisciplinary team, the Social Worker helps ensure each resident receives individualized support based on their needs, preferences, and goals.

Key Responsibilities
- Complete psychosocial assessments and support individualized care planning.
- Serve as an advocate and resource for residents and families.
- Coordinate discharge planning and safe transitions of care.
- Participate in care conferences and collaborate with the interdisciplinary team.
- Support residents with adjustment, behavioral health, grief, and other psychosocial needs.
- Assist residents and families with community resources and supportive services.
- Promote resident rights, dignity, choice, and quality of life.
- Address resident and family concerns and assist with conflict resolution.
- Maintain accurate and timely documentation in accordance with regulatory requirements.
- Support facility quality improvement, compliance, and survey readiness efforts.
Requirements
Degree in Social Work, Human Services, or a related field preferred.
Licensed Social Worker (LSW) preferred, but not required.
Previous experience in skilled nursing, long-term care, healthcare, or social services preferred.
Strong communication, organization, and interpersonal skills.
Ability to work collaboratively with residents, families, and the interdisciplinary team.
Compassionate, resident-centered approach with strong problem-solving and advocacy skills.
